You also need to use this service if you require manual work doing to the document such as showing more than one name change, etc.

Applying for an Adult or Kids Deed Poll

Adult Application: If you are 16 year old or over, then you need to apply for an adult Deed Poll document. As an adult, you do not need the consent of anyone to change your name.  The adult Deed Poll allows you to officially change your name, a part of it, all of it, or just some spelling in it. You can also change your social title using the Deed Poll, for example from Miss to Mrs or Ms, etc.

Child Application: IF the person is under 16 years of age, then a Parent or Legal Guardian needs to apply for then using the Child Deed Poll Application form. The child or kids Deed Poll allows you to officially change the name of the child, a part of it, all of it, or just some spelling in it.
